Catholic but not on the voting list?
You can contact Anthony to determine if you are on the Catholic voters list and if you are not you will need to fill out one of the Application to Amend Voters List form.

Forms should be available at the polling station or contact Anthony Danko for one of these forms.

"Where a person's (qualified elector) name does not appear on the Voters' List, or the information is incorrect, the deputy returning officer and/or designate (DRO) will issue the person an "Application to Amend Voters' List" form. The DRO will require the person to complete the form and to provide proof of identification in order to verify qualification (picture identification and proof of residence is required). Once the "Application to Amend Voters' List" form is completed and accepted, the DRO will add or correct the person's information, and issue the voter a ballot. It is recommended that revisions to the Voters' List be requested prior to voting day, in order to avoid line ups at the voting location." - Town of Oakville website.
